Note: handicap will not be used in scoring, but when filling out foursomes we will attempt to
match golfers with similar handicaps as far as possible.

Transportation:  We need to know if you plan to go directly to the golf course from your home, or if you will arrive at the Tradewinds Beach Resort a day early and want transportation to the course from the hotel the morning of the tournament.  Please check the appropriate following response:

Fee:  $xx.00 per golfer includes greens fee, 1/2 cart, lunch, and prizes for the winners.

Format:  Shotgun start at 0830 on Tuesday, Month, dayt.

Awards:  low gross wins the rotating Florida MOAA Golf Trophy.  Other scoring is by a modified Callaway scoring system.  Prizes given for "Closest to the pin".

Luncheon will be held at Beautiful Club House immediately following completion of the golf.

If you have arranged a foursome with others, please list their names and home town below so we can plan it for you.

We will send golfers precise instructions on how to get to the Beautiful Golf Club.
